Consent to Tattoo Procedure - Because Great Art Comes with Great Responsibility
Before we get started on your masterpiece, we need to make sure we’re all on the same page. This is the official (but still friendly) tattoo consent form—please read it carefully and sign below. By signing, I confirm that all my questions have been answered, and I agree to the terms outlined.

I confirm that I am 18 years or older (or 16+ with a parent/legal guardian present).

I understand that tattoos are permanent (yep, forever) and that removing one later can be expensive, painful, and might not be fully effective.

I do not have any medical conditions that could interfere with the tattoo healing process (such as hemophilia, diabetes, epilepsy, skin conditions, or allergies to ink or latex).

I understand that allergic reactions to tattoo pigments or processes are possible, and I accept this risk.

I am not pregnant or nursing or under the influence of drugs or alcohol.
Aftercare & Responsibility

I understand that proper aftercare is my responsibility, and failing to follow instructions may lead to infection, fading, or other complications. I have received aftercare guidelines and agree to follow them while my tattoo heals.

I accept that neglecting my tattoo—such as excessive sun exposure, swimming in unsafe water, or improper care—may affect the results, and any necessary touch-ups due to my own actions will be at my expense.

I also acknowledge that skin treatments, laser hair removal, plastic surgery, or other procedures may alter my tattoo’s appearance.
The Artistic Agreement

I have reviewed and approved the design, placement, and size of my tattoo.

I trust my artist to bring my vision to life, understanding that tattoos are hand-done art and slight variations may occur.

I acknowledge that color and design may heal differently based on my skin tone, healing process, or natural tattooing variations.

If multiple sessions are needed, I understand that touch-ups may require an additional fee.
Photo Release
